A company is looking for an RN Care Manager - Ambulatory Care
Management, Hybrid-Remote.
Key Responsibilities
Function as a clinician, advocate, and educator to manage care and
resources effectively
Develop person-centered care goals and individualized care plans to
promote patient self-management
Identify and prioritize potential problems, engaging in
problem-solving processes as needed
Required Qualifications
Current unrestricted California RN license
BSN Degree
5 years of Case Management experience
Proficiency in computer skills and various healthcare
applications
Ability to adapt to changing circumstances and manage multiple
priorities
